Jack entered  the Service from New York with service-number # 2237245.

 

Jack was KIA when the ship was attacked by many enemy Dive Bombers and Sunk near Tjilatjap, Java, on March 1, 1942, he is honored with a  Purple Heart, Combat Action Ribbon, Asiatic-Pacific Campaign Medal and the WWII Victory Medal.

 

Jack is buried/mentioned at Manila American Cemetery and Memorial  Manila, Metro Manila, National Capital Region, Philippines.
